At Oasislife Care, we believe great care starts with understanding you – your needs, preferences, and goals. That’s why we begin with a warm, no-obligation conversation – either with you, your family, or both – to explore how we can support you to live well at home or in supported living.
During this initial meeting, we’ll talk through key areas such as:
- Health & medical history – including any diagnoses, medications, and health professionals involved in your care.
- Daily Living needs – such as personal care, mobility, meals, routines, and any help needed at home.
- Emotional and social wellbeing – including your interests, support networks, mental health, and companionship needs.
- Cognitive and behavioural needs – to understand how you process information, communicate, and any behaviours that require support.
- Risk and safety – identifying any potential risks at home or in your care, including falls, self-neglect, or safeguarding concerns.
- Legal and advocacy matters – such as capacity, consent, Power of Attorney, or involvement of advocates.
- Involvement of other professionals – making sure we work in partnership with social workers, nurses, and therapists already supporting you.
You’ll also have the chance to share what’s important to you, ask questions, and tell us how you’d like your support to look and feel. Your care plan will be fully tailored to your routine, lifestyle, and values.
